How Patients Can Support the Pharmacy Team
Patients can assist the Pharmacy Department in delivering safe and effective care by:
- Bringing an up-to-date list of all current medications to assist in compiling a Best Possible Medication History (BPMH).
- Bringing any medications prescribed or initiated by a hospital consultant, including “High Tech” medications.
- Providing the name and contact details of their regular community pharmacy, to support communication and continuity of care following discharge.
- Informing pharmacy or nursing staff if their medications are usually supplied in pre-packed blister packs.
