Our Medical Social Workers assess the wider impact of illness or injury on patients and their families, and plan appropriate support and welfare services they need to move forward.

- Safe, confidential and supported service
- Facilitate discharge plan
- Liaison with community services
- Information and advice
- Support in accessing social welfare services and community supports
