Our Patient Pathways
The hospital provides rehabilitation services across six distinct care pathways and these include:
Integrated Care for Older Persons (ICPOP) Pathway with the Mater Hospital
Clontarf Hospital has 16 ICPOP beds under the clinical governance of an ICPOP Consultant Geriatrician. Patients aged over 65 years with complex comorbidities are admitted to this pathway either directly from the Mater Hospital Emergency Department Frailty Team or directly from home. In 2025, this pathway supported 133 admissions. 81% of patients were admitted to Clontarf Hospital directly from the Mater Hospital Emergency Department and 19% of patients were admitted directly from home. These beds support admission avoidance for the Mater Hospital and also prevent Emergency Department presentations for those admitted directly from home.
Older Persons Rehabilitation Unit with the Mater Hospital
This 32-bed unit provides intensive interdisciplinary rehabilitation to patients aged over 65 years who are transferred from the Mater Hospital following an acute admission for illness or injury. Patients on this pathway have defined rehabilitation goals with the aim of discharge to home. The service is led by a Consultant Geriatrician. 301 patients were supported by this pathway in 2025.
Older Persons Rehabilitation Unit with Beaumont Hospital
This 32-bed unit delivers intensive interdisciplinary rehabilitation to patients aged over 65 years who are transferred from Beaumont Hospital following an acute admission for illness or injury. Patients on this pathway have specific rehabilitation goals focused on discharge to home. Clinical leadership is provided by a Consultant Geriatrician. 261 patients were supported by this pathway in 2025.
Specialist Rehabilitation Pathway with the Mater Hospital
In September 2024, a pilot for a five-bed unit to provide Level 2b specialist rehabilitation for patients aged 18 years and over admitted to the Mater Major Trauma Centre with life-changing injuries was commenced. The service continues to operate under the governance of a Consultant in Rehabilitation Medicine and now includes neurorehabilitation. This pathway supports a reduction in acute hospital bed-days for the Mater Hospital and a reduction in waiting lists and admission avoidance for the National Rehabilitation Hospital (NRH). In 2025, 22 patients were admitted to this pathway.
Specialist Rehabilitation Unit with Beaumont Hospital
This 16-bed unit provides Level 2b specialist rehabilitation, including neurorehabilitation and amputee rehabilitation, for patients aged 18 years and over transferred from Beaumont Hospital. Care is delivered under the clinical governance of a Medical Consultant. This pathway supports a reduction in acute hospital bed-days for Beaumont Hospital and a reduction in waiting lists and admission avoidance for the National Rehabilitation Hospital (NRH). 130 patients were admitted to this pathway in 2025.
Orthopaedic Rehabilitation Pathway
This 43-bed pathway provides rehabilitation for patients aged 18 years and over following orthopaedic surgery for elective and trauma patients. Patients are referred from multiple acute and scheduled care hospitals and remain under the clinical governance of their referring Consultant Orthopaedic Surgeon.
