What to bring with you when you are admitted

Your rehabilitation team will support you to regain your strength and independence. They will encourage you to be active as soon as possible. To help with this, please bring:

  • Comfortable clothes for day and night
  • Well-fitting, closed shoes or trainers
    • Slippers should have non-slip soles and a closed back for safety
  • Underwear and socks
  • Any walking aids, hearing aids, communication aids or glasses you use
  • Toiletries, such as:
    • Toothbrush and toothpaste
    • Shampoo and shower gel
    • Hairbrush or comb
    • Shaving equipment, if needed
  • Medication List and Medications
    • Up-to-date list of all current medications to assist in compiling a Best Possible Medication History (BPMH).
    • Bringing any medications prescribed or initiated by a hospital consultant, including “High Tech” medications
  • Name and contact details of their regular community pharmacy, to support communication and continuity of care following discharge.

Valuables:  Please do not bring large amounts of money, jewellery, or other valuable items.  If you cannot arrange for a family member or friend to take your valuables home, please give them to a nurse.  The nurse will store them safely in the hospital safe.

Discharge Planning

Your multidisciplinary team will work with you to set your rehabilitation goals and review your progress.  Discharge planning is an important part of your care and will begin when you are admitted.

  • Your team will agree an estimated discharge date (EDD) with you
  • Meetings with you and your family will be arranged when needed
  • A home visit may be carried out to assess your home and support a safe discharge
  • With your consent, your team will contact community services and your GP to plan your discharge
  • If you complete your rehabilitation programme and cannot return home, your team will work with you to arrange a transfer to step-down care
